My ENB realism pack runs super smooth, especially after messing with the enblocal.ini a bit. Like I said, I just used the generic 4GB patcher, patched FalloutNV.exe, FalloutNVlauncher.exe and nvse_launcher.exe (basically just everything because I didn't know what I was doing ), and I haven't had any problems since.
